    LUNA'S TACOS AND TEQUILA WINDSOR, 1264 Automation Dr, Windsor, Oct. 5 — An employee put multiple pans of raw chicken into the oven with bare hands, then put on gloves and touched ready-to-eat tortillas without washing; dirty dishes were also handled with gloved hands before clean dishes were put away without changing gloves or washing. Cooked adobo chicken was at 92F while cooling at room temperature atop the oven, with no measured temperature change after 20 minutes (the operator said it had been out for less than an hour), and pans of cooked chicken were also left at room temperature without additional cooling methods; crema, house-made adobo sauce, coleslaw dressing, salsas and sour cream in a cook-line cooler at 56F measured 50-53F (the operator said they had been in the cooler overnight); two containers of adobo sauce were past their discard date, a gallon of milk held more than 24 hours was undated, fully thawed tuna was in reduced-oxygen packaging in a cooler drawer, shrimp was thawing in ambient-temperature water in the prep sink, and saturated wiping cloths were on kitchen and bar prep surfaces. A stock pot blocked the hand sink nearest the oven during food preparation, and there was no sign reminding employees to wash their hands after using the restroom.

    ABARROTES LA CRUZ, 2004 8th St, Greeley, Oct. 8 — Employees could not answer food-safety questions and did not demonstrate food-safety knowledge; staff were told to obtain training within 10 days. Dozens of bags of frozen tomatillos, chorizo, chocolate-covered strawberries, bananas and banana leaves grown, cooked and/or packaged at the operator's private home and garden were found on the sales floor; raw chorizo was stored above queso fresco; frozen packages of chorizo, tomatillos and banana leaves were at ambient temperature without temperature control while the operator cleaned condensate and ice buildup from the freezers; dozens of frozen tomatillo and chicken bags were unlabeled (the operator said the chicken was bought at a grocery store and packaged on site); and frozen raw chicken thighs were in non-food-safe grocery bags.

    THE TAVERN AT SAINT MICHAELS, 2918 67th Ave, Greeley, Oct. 5 — Employees did not demonstrate food-safety knowledge, and the facility did not have a plan that met requirements for cleaning up vomiting or diarrheal events. The sink beside the dish pit had no hot water because staff had turned it off due to a leak, and staff said they had called a plumber; two Soda Stream bags in use were on the floor, which was wet from the dish-pit area; six house-made dressings containing foods requiring temperature control were at 56F in a shallow pan of cold water and dated 9/27 (staff said they put them out daily but could not verify how long they had been out of temperature control); 10 bags of par-cooked wings dated 9/27 were held beyond their discard date; and foods in the walk-in cooler were at 43F (staff said it had been serviced the day before because it was too iced over and its dial had not been reset).

    WING SHACK JOHNSTOWN, 21 N Parish Ave, Johnstown, Oct. 7 — An employee came into the kitchen from break, put on gloves and began preparing a sandwich without washing hands; the hand sink near the dish pit had no soap or paper towels. Shredded cheddar, sliced tomatoes and whipped cream cheese in the prep cooler were at 45F, with products stored above the fill line and/or double-panned.

    SUBWAY 21455, 4835 W 10th St, Greeley, Oct. 8 — The person in charge did not have a certified food protection manager, could not provide certification, or the certified manager was not present at the inspection. An employee came in from outside, put on gloves and made a sandwich without washing hands; sliced ham, salami and turkey in the front prep cooler measured 46-48F above the fill line (the cooler had been stocked an hour earlier); beef was thawing at 55F on the prep counter, and the prep-line hand sink leaked, with water puddling outside its compartment.

    DOUBLE SPEED (Cosmic Market), 2730 23rd Ave, Greeley, Oct. 8 — There was no documentation that employees had been trained on their illness-related responsibilities, and a written employee illness policy is required; per the operator, cat litter was used to clean bodily fluids, and the facility did not have a plan that met requirements for cleaning up vomiting or diarrheal events. The hand sink by the Pepsi machine had no soap, though customers were seen washing their hands there before food handling; no sanitizer was available for the hot-dog rollers and tongs; the ice machine had visible black grime/mold in its crevices (the operator said the ice was used in the Pepsi machine and sold in bags); two open packages of ready-to-eat tornados in the walk-in cooler were undated though held for more than a day; and no food probe thermometer was on site.

    LONCHERA SANTA CRUZ LLC #2, a mobile unit, Oct. 9 — The person in charge did not have a certified food protection manager, could not provide certification, or the certified manager was not present at the inspection; contaminated hands were washed in the food-preparation sink, and a gloved employee wiped his face with his shirt before continuing to prepare food with utensils on the flattop. Cooked beans and cooked chicharrones in the steam well measured 117-118F and, 15 minutes later, 113-120F; an employee said they had been cooked the night before and placed in the steam well 30 minutes earlier; raw beef in the bottom-left cooler drawer was at 44F, and an employee said it had been transported from the commissary 30 minutes earlier.

    QDOBA MEXICAN EATS #2198, 4626 Centerplace Dr, Greeley, Oct. 7 — An employee came from a break in the lobby, served herself chips without washing her hands, then took the chips back to her table. The hand sink in the bar contained used cooking utensils.
