RICAS PUPUSAS A, a mobile unit, Oct. 1 — Staff could not demonstrate food-safety practices, and water at the only hand sink measured above 130°F, preventing employees from washing their hands; the unit’s only thermometer read 30°F for the air while the inspector’s calibrated thermometer read 100°F. Sealed pork fat contained black fuzzy mold; mozzarella mixtures measured 45-50°F, cooked beef 49°F and shredded lettuce 50°F, sanitizer exceeded 200 ppm, and a fly strip hung over a prep table.

DRAGON INN, 101 2nd St, Eaton, Oct. 1 — The inspector found live larvae in cooked broccoli and saw it handled over uncovered cooked lo mein; the noodles measured 50-54°F, and shredded cabbage was 45°F. Live flies landed on kitchen food-contact surfaces, a used fly swatter hung among clean utensils, saturated fly strips hung above to-go containers and clean dishes, and sanitizer measured both above 200 ppm and below 50 ppm.

KFC, 4335 City Centre Dr, Firestone, Sept. 30 — Employees handled raw chicken or beef and then touched fryer or cooler handles without washing their hands or changing gloves; an employee also used the handwashing sink to dump a drink. Cooked chicken measured 48-50°F, and an employee said it had been put in the walk-in cooler the night before; coleslaw measured 46°F, and an employee said it had been prepared two hours earlier, while sanitizer measured above 400 ppm.

CIRCLE K #2709082, 6109 Firestone Blvd, Firestone, Sept. 30 — No certified food protection manager was present, and the facility had no documentation that employees were trained on illness reporting. An employee touched their face and pants and drank from a personal beverage before handling hot dogs without washing their hands; a can was severely dented, and in-use utensils were cleaned once a day.

Roasted corn and chicken salad were past their discard dates; cooked chicken, pico and coleslaw had incorrect date marks, cleaners were stored above soda refills and single-service items, and bar sanitizer was below 150 ppm.

An employee said the cheese had been put in the unit 2.5 hours earlier and the hot dogs cooked an hour earlier; two opened packages of precooked hot dogs had no date marks, and an employee said they had been opened four days earlier.
